Information for Completing the Practitioner Application
There are two versions of this form: an e-mail version and a paper version. The e-mail version can be filled in on-line. You can save the completed form on your computer and/or print it. The paper form must be printed. It should be filled out in pen clearly and legibly. Both version of the form must be signed before being sent to the Registry.
Practitioner Application Instructions
Please follow the instructions below when completing your application.
Fill out all information on the application.
Make a copy of the completed application for your personal files.
Make copies of all certificates, diplomas, transcripts, etc. related to the information you completed
on the application:
GED or high school diploma
DCF Child Care courses
CDA (National, Florida equivalency and renewals)
Director’s Credential
College degrees
Any college courses completed in the last two years
Any workshops or trainings attended in the last two years
Professional activities
Helpful tips about certificates, diplomas, transcripts, etc.
a) Documentation of DCF Child Care classes (20-hour, 10-hour, etc.) may be obtained at
http://www.Myflorida.com/childcare/; click on
"Training Requirements," then click on "View My Transcript" and follow the log-in instructions.
b) High school diplomas or GEDs may be documented on Palm Beach State College or other college transcripts.
If you have taken classes at Palm Beach State, go to http://www.palmbeachstate.edu/x3997.xml and follow the instructions to log in and view
and print an unofficial transcript. Check transcripts from other colleges for documentation of high school
diploma or GED.
c) All non-credit/informal training (workshops, conferences, etc.) certificates MUST contain the following
information and be legible: your name, the date and title of the training, the trainers name and agency
(if applicable), and the number of training hours or CEUs earned. Certificates that do not contain all of
this information cannot be accepted for documentation of training.
Mailing your application and receiving your Registry certificate
Mail the completed original application and all copies of documentation to the Registry office:

A copy of your Registry certificate and a print-out of your training record will be mailed to you within 4 weeks.
